Locum HEM/ONC APP – Northwest Arizona
License Status Required — Active AZ license required for submission; not willing to wait for license
Certifications Required (BLS/ATLS/PALS etc.) – None – active DEA required
Shift times- Tuesday–Friday, 7:00 AM – 5:30 PM
Dates- Start date 11/3/2026; assignment 6 months+
Census- ~12 patients/day, 30-minute appointments (consults and follow-ups)
EMR- Meditech, Aria
Credentialing Timeframe? 60–90 days; payer credentialing (90 days) must be completed before provider can start
What does a typical day look like? Outpatient clinic Tuesday–Friday, 7a–5:30p; ~12 patients/day in 30-minute consult/follow-up slots. Supported by personal MA, scheduler, case manager, referrals coordinator, admitting staff, clinical trials nurse, navigation nurses, infusion nurses, on-site pharmacists, remote dietician, authorizations staff, oral medications staff, and LNA. Tumor board held 1st and 3rd Tuesdays, 12–1 PM; provider expected to present cases.
